L4 (Lunch, Launch, Listen and Learn)

We are excited to introduce our new partnership with Community Resource Network (CRN) the Community Information Exchange (CIE) from western Colorado. We will provide you with the opportunity to see a system demonstration and ask questions of the CRN team.

We are also seeking your input on expanding CommuniCare across the entire state and having you share with us who you work with and how you support them. Please bring any relevant information on your priority (or target) populations and your services that could impact Social Drivers of Health (SDOH).
